4-H Shooting Sports Instructor Certification Survey


Ohio 4-H Shooting Sports Instructors maintain certification in each discipline by teaching in that discipline at least ONCE every two years.  Please complete and return the form below so we can update your certification records. Please complete the form by January 1, 2010.

Ohio 4-H Shooting Sports Instructors maintain certification in each discipline by teaching in that discipline at least ONCE every two years.  You must be an active volunteer within your county every year or your county office will remove you from their active roster, and in that light will disallow you to participate as an active 4-H certified shooting sports instructor. This interruption of inactivity in your respective county could cause the county to ask you to reapply through the 4-H volunteer process. Please complete and return the form below so we can update your certification records. Please complete the form by January 1, 2010.
